Many of our patients do not have a primary care provider often because most providers in the area are booked out 6-12 months. To meet the needs of our patients, we have added primary care to our Brentwood and Berry Farms locations. This expands our services from just illness and injury, to a full-range of preventative care and the management of chronic conditions. Ages 12 and up. Make your appointment online!

Elizabeth Jasarevski, FNP-BC

Primary Care Provider

Elizabeth is a dedicated nurse practitioner with 7 years of primary care experience. A Tennessee native, she earned her Master of Science in Nursing from Belmont University in 2016. Elizabeth is committed to providing compassionate, patient-centered care to her community.

Chandler Hardman, MD

Supervisory Physician

Dr. Chandler Hardman is the supervisory physician for Primary Care at Physicians Urgent Care. He received his Doctor of Medicine from the Medical College of Georgia and completed his internal medicine residency at Vanderbilt University. Dr. Hardman has over 30 years of experience in primary care and provides guidance to our team by reviewing charts, consulting on cases, and assessing services and clinical processes.

David R. Bloom, MD

Primary Care Provider

Dr. David R. Bloom earned his Doctor of Medicine from the State University of New York at Buffalo. With over 30 years of primary care experience, he has spent the past decade practicing in Huntington Beach and Laguna Niguel, California, where he also served as Medical Director.
